Abstract
This Doctor of Nursing Practice (DNP) quality improvement evaluation project addressed inconsistent use of the Patient-Centered Daily Care Plan at a local acute care hospital. The problem was a gap in compliance with California Title 22 requirements, creating risk for survey deficiencies and limiting interdisciplinary communication. The purpose of this project was to evaluate whether the revised daily care plan improved individualized documentation and organizational compliance. De-identified aggregate data from pre-implementation (August–October 2023) and post-implementation (June–August 2024) periods were analyzed descriptively. Results showed a meaningful improvement in checklist completion, with compliance increasing from 94.8% to 97.3%. These findings demonstrated stronger Title 22 adherence, improved interdisciplinary communication, and enhanced survey readiness. The implications for nursing practice and social change include sustaining patient-centered documentation, promoting regulatory accountability, and supporting equitable, individualized care delivery across acute care settings.
