Abstract
Within this executive summary, the type of project discussed is an organization change project to address a health issue in chronic disease management, which is hypertension. Nearly 116 million American adults are affected by hypertension, which remains a leading force for preventable hospitalizations, emergency department visits, and cardiovascular-related mortality in the United States. Unexpected readmissions due to hypertension create system-wide strains, increased financial burden and decreased patient satisfaction. Hence, the question: In adults with hypertension, does remote home blood pressure monitoring, compared with standard office-based follow-up, reduce the rate of urgent care visits and hospital admissions over a 30-day period? Evidence was obtained from the doctoral project’s patient-level clinical and administrative records, including hospitalization and discharge data, follow-up documentation, HBPM participation or monitoring records, and 30-day readmission records. The project’s evidence supports evaluating HBPM as a transitional-care strategy because it creates a mechanism for earlier recognition of persistent or worsening hypertension after discharge. The HBPM group had fewer 30-day readmissions than the standard office-based follow-up group, indicating a favorable short-term outcome for HBPM-supported follow-up. Remotely monitoring blood pressure is a low-cost, collaborative-care strategy that extends surveillance beyond the in-person practice. Evidence supports that high blood pressure monitoring, when incorporated with telemonitoring and follow-up, reduces the possibility of preventing increased urgent care and emergency room visits by establishing timely detection of blood pressure decline before it becomes severe, requiring hospitalization.
