Abstract
This doctor of nursing practice (DNP) executive leadership project is a practice-focused quality improvement initiative to reduce avoidable 30-day hospital readmissions in a skilled nursing facility by implementing the INTERACT communication tools, SBAR and STOP & WATCH. The practice problem is the lack of a standardized communication process for recognizing changes in resident conditions, contributing to preventable hospitalizations. The project's purpose was to develop an evidence-based communication process that promotes early recognition and timely provider intervention. The practice-focused question asked whether implementing INTERACT communication tools would improve communication and reduce avoidable acute care transfers. A focused literature search was conducted. Current evidence and national practice guidelines support standardized communication to improve resident safety and reduce preventable hospitalizations. The project resulted in an implementation plan that includes INTERACT tools, staff education, leadership oversight, and Quality Assurance and Performance Improvement or QAPI monitoring. The project strengthens nursing practice through evidence-based communication and promotes equitable, resident-centered care to improve resident outcomes, promoting positive social change.
