Abstract
Patients with chronic conditions have the tendency to return to the hospital. Staff education to address high-rate re-hospitalization among patients with chronic conditions is the focus of my Doctor of Nursing Practice project. This is a significant practice problem, since patient outcomes and increased healthcare costs is compromised. Regarding re-hospitalization prevention and ultimate aid in reducing readmissions among people with chronic conditions, the project was used to enhance the knowledge of the nursing staff and competence in transitions of care and discharge planning. The project included 10 nurses who are regularly involved in discharging patients in the medical outpatient clinic. Participants answered questions anonymously to a pre-test which was conducted where a 10-question questionnaire was issued. One week after the education session, a post-test of the same questionnaire was conducted. A ttest analysis was conducted on MS Excel and the results indicated a statistically significant improvement in post-test scores: t(9) = -8.82, p = <.001. In discharge planning and transitions of care with the project, it demonstrated that structured education can indeed improve the nurses’ knowledge. During the discharge process a discharge checklist to be used. To support long-term nursing practice major recommendations, including staff training. These recommended interventions support positive social change by reducing health disparities in chronic disease management.
