Abstract

At the practice site, older residents face inconsistent and untimely blood sugar assessments, contributing to avoidable complications such as hypoglycemia, hyperglycemia, and hospitalizations. Many caregivers lack standardized training in blood glucose monitoring, which poses a significant risk to residents with diabetes or prediabetes. To address this practice problem, a staff education program on blood glucose monitoring protocols was initiated. The practice-focused question was the following: Will staff knowledge and confidence increase after participating in an education program on monitoring blood glucose levels? As part of the education program, staff received evidence-based instruction on proper techniques for blood glucose testing, recognizing abnormal glucose values, appropriate assessment timing, and responding to out-of-range results in accordance with current clinical guidelines. The project utilized pre- and postintervention assessments consisting of 10 questions including multiple choice, true/false, and short answer. Data was analyzed using the learning gain percentages formula and the mean calculated learning gain was 71.17%. Among the 10 participants who completed the education program, the pretest mean score was 56% and the posttest mean score was 88%. Recommendations include sustaining the training program through ongoing competency evaluations and integrating digital monitoring tools to further streamline workflow and data collection. This project highlights the critical role of nurse-led initiatives in chronic disease management in long-term care settings. The project promotes safe, evidence-based practices and supports a culture of accountability and continuous learning. The project reinforces principles of diversity, equity, and inclusion by ensuring all residents receive equitable and high-quality diabetes care regardless of caregiver background or shift assignment.
